Revitalize PT and Fitness Clinic is seeking a dedicated and experienced Physical Therapist to join our team in Yuba City, California. This full-time position plays a crucial role in delivering high-quality care to patients in home care settings, enabling them to recover and regain their independence. Work Environment

The work environment is primarily onsite within patient homes, allowing for a personalized approach to rehabilitation. As a Physical Therapist at our clinic, you will interact directly with patients, their families, and other healthcare professionals in their daily environments. Key Responsibilities

• Conduct thorough assessments of patients, evaluating their physical condition and mobility limitations to create effective treatment plans tailored to their specific needs.
• Implement evidence-based rehabilitation programs, employing various therapeutic modalities and exercises to improve patients' physical health.
• Educate patients and their families on treatment protocols, addressing questions and providing information for follow-up at home.
• Document all patient interactions and treatment progress in accordance with regulatory and accreditation standards, ensuring accurate record-keeping in our electronic medical records (EMR) system.
• Collaborate with other healthcare professionals, sharing insights about patient progress and problems to enhance overall care delivery.
• Regularly update treatment plans based on progress, reassessments, and ongoing evaluations to align with patient goals.
• Maintain cleanliness and safety of all therapeutic equipment and ensure the therapy space is organized and compliant with health regulations.
• Foster a culture of continuous improvement by participating in team audits and providing constructive feedback on best practices and documentation improvements.

Patient Population / Client Focus

In this role, you will work with a diverse range of patients, including individuals recovering from orthopedic injuries, surgeries, sports-related ailments, and chronic musculoskeletal conditions. Our clientele often consists of older adults, athletes, and patients requiring rehabilitation for various health-related issues. Each patient presents unique challenges and goals, allowing you to utilize your expertise to craft tailored approaches that promote recovery and enhance their quality of life.

Qualifications

• A valid Physical Therapist license in the State of California, in good standing.
• Completion of an accredited Physical Therapy program resulting in a Bachelor's degree or higher.
• National Provider Identifier (NPI) number.
• Strong foundation in orthopedic rehabilitation methodologies and an understanding of musculoskeletal conditions.
• Ability to develop and implement individualized treatment programs based on evidence-based practices.
• Excellent verbal and written communication skills, with the capability to educate and motivate patients effectively.
• Ability to thrive both independently and as part of a collaborative, multidisciplinary team.

Preferred Experience

• 2+ years of hands-on experience in a clinical setting, specifically dealing with orthopedic and sports rehabilitation.
• Familiarity with electronic medical record systems and documentation best practices.
• Experience working in a home care environment is a plus.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $125,000 annually, commensurate with experience. Additionally, we provide a comprehensive benefits package, including:
