Job Responsibilities

• Conduct comprehensive evaluations for patients in both inpatient and outpatient settings, crafting personalized treatment plans
• Implement therapeutic interventions focused on enhancing mobility, strength, balance, and overall functionality
• Collaborate closely with nurses, physicians, case managers, and various rehabilitation professionals to enhance patient outcomes
• Guide patients and their families on setting therapeutic goals, strategizing discharge planning, and implementing home exercise programs
• Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with systems and regulatory standards
• Engage in interdisciplinary team rounds and conferences to promote unified patient care

Qualifications

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Licenses/Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (e.g., neurological, geriatric) considered advantageous but not mandatory
