Primary Care Physician

Oak Street Health seeks a part-time Primary Care Physician to deliver high-quality, preventative care to adult Medicare patients in underserved communities. In this patient-centered, team-based role, you will manage chronic conditions, perform wellness visits, and coordinate care with nurses, social workers, and other clinicians. Responsibilities include thorough clinical assessments, evidence-based treatment, and clear documentation in the EHR. You’ll help drive quality improvement, support population health initiatives, and contribute to an inclusive, collaborative culture focused on equity and community impact.
