Active Kentucky PT license or ability to obtain state licensure prior to start. Current CPR/BLS certification. Experience with Home Health setting, including start‑of‑care evaluations, recertifications, and discharges. Familiarity with Home Health documentation systems such as HCHB or PointClickCare/Pointcare. Ability to travel within coverage areas: Boyle, Garrard, and Madison counties. Willingness to work assigned shift schedule. Minimum education requirement: Associate degree. Preferred Prior home health travel experience and familiarity with mileage reimbursement processes between visits. Assignment Details Facility Type: Home Health. Shift Name: Evenings. Shift Details: Evenings 2000-0400. Days: 5x8 schedule. Coverage Areas: Boyle, Garrard, and Madison counties. Productivity Expectations: 30 points (client‑specific). Weekly Hour Guarantee: 40 hours. On‑call/Weekend Requirement: No call requirement. Mileage reimbursed between visits from first patient to last patient. Required Certifications: PT License and CPR.
