Cadence Care Network has an opening for an Early Intervention Service Coordinator.


Responsibilities Include:
-Provide support and consultation services to referred families of infants and toddlers with disabilities or developmental delays in Franklin County, OH.
-Take primary responsibility for the development and implementation of the Individual Family Service Plan(IFSP).
-Coordinate all necessary services/treatments, referrals, and assessments.
-Work in collaboration with community providers and provide referrals for additional services the family may need.
-Attend weekly team meetings with community providers to ensure continuity of care. Attend trainings with community partners and stakeholders as it relates to Early Intervention.

Qualifications:
-Bachelor's degree in human service related field.
-Experience working with children ages 0-3.
-Valid Driver License, Auto Insurance and the provision of a vehicle for work purposes.
