Field-Based Community Health Advocate
Optum
UnitedHealth Group is seeking a Field Based Community Health Worker to actively engage health plan members in Greenville, SC and surrounding communities. The role involves home visits, care coordination, and education on plan benefits, with a focus on improving access and outcomes. The position is full-time with a 40-hour workweek, typically Monday–Friday, 8:00am–5:00pm EST. Telecommuting options may be available, especially for SC-based residents.
