RN Case Manager - Hospice

We are seeking an experienced Registered Nurse (RN) Case Manager to provide compassionate hospice care, manage patient needs, coordinate services, and support patients and families throughout the hospice journey.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Coordinate hospice services with physicians and the interdisciplinary care team.
• Administer medications and provide skilled nursing care.
• Monitor patient condition and communicate changes appropriately.
• Provide wound care, catheter care, and other nursing interventions as needed.
• Educate patients and families on medications, symptoms, and end-of-life care.
• Maintain accurate and timely clinical documentation.
• Coordinate follow-up care and support continuity of services.
• Travel within the assigned service area to visit patients.

Requirements

• Active RN license in good standing.
• Minimum 1 year of hospice experience required .
• Strong understanding of hospice philosophy and patient-centered care.
• Excellent communication and care-coordination skills.
• Ability to work independently and manage multiple patients.
• Valid driver’s license, reliable transportation, and ability to travel locally.
