Partnering with residents, family members, and the staff; this position will be responsible for supporting the social service needs of residents, in addition to ensuring the highest quality of resident care and customer service in our community.What we offer:A culture of diversity, inclusion, equity and belonging, which builds on our mission, vision and valuesMedical, dental and vision packages, including an annual reimbursement for qualified wellness expenses, personal health coaching and telemedicine optionsPTO Plans, PLUS company paid volunteer hours for eligible team members, in accordance with applicable state law401k for all team members 18 and over with a company 3% matchOnsite medical centers, providing wellness visits and sick care for all team members over 18 years of ageFree access to our on-site Team member Health and Well-Being Centers, plus Well-Being programs, tools and resources for you and your immediate family membersEducation assistance, certification reimbursement and access to over 6,000 courses through our online learning library, designed to enhance your current skills and build new onesGrowth Opportunities – grow with the company as we open new communities and expand on our existing ones!How you will make an impact:Leading a holistic, person-centered approach to assessment and service/care planningConducting pre-residency assessments and screening of potential residents and making recommendations for an appropriate level of residencyResponsible for evaluating the psychosocial and activity of daily living (ADL) needs of residents and coordinating care and services as needed.Working with the interdisciplinary team to communicate with residents and families regarding changes in the plan of care and residents' conditionProviding individualized psychosocial support to residents and their families via family meetings or support groups and assists through the transition process to a higher level of carePartnering with Independent Living and Post-Acute Social Workers to develop support groups and educational programming opportunities for residents and family members, as well as working together when transitioning between levels of careCompensation: Salary range $75k-$90k with eligibility for annual bonus.What you will need:Minimum of 1-year related experience and/or training in Long-term care/assisted living preferred. Education will be considered in lieu of experience.Experience with the elderly and understanding of the dementia process and its impact on daily living and activity programming.Must have knowledge of the geriatric population and the aging process, including the physical, psychological and social needs of the elderly.Meets state-specific requirements for management certification of assisted living and/or personal care facilities (within 90 days of hire)National Certification Council for Activity Professionals (ADCP or ADC) as required by the StateACC (Activity Consultant Certified), CDP (Certified Dementia Practitioner), or CTRS (Certified Therapeutic Recreation Specialist) is preferred.Wind Crest is a beautiful 84-acre continuing care retirement community located in Highlands Ranch, Colorado, just minutes from Denver.
