Hospice Case Manager

Allina Health in Inver Grove Heights seeks an experienced Hospice Case Manager RN for a day shift. This position offers 8-hour shifts, 40 hours per week, and an every-4th weekend rotation within a union-represented department. You will perform comprehensive assessments, plan and coordinate care, and partner with a multidisciplinary team to ensure high-quality hospice services. The role requires an Associate's or Vocational degree and 2+ years RN experience, MN/WI licensure eligibility, and BLS #J-18808-Ljbffr
