Behavioral Health Specialist

Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adult Medicare patients in underserved communities. In this bilingual English-Spanish role, you will conduct psychosocial assessments, deliver individual and group therapy, and create evidence-based treatment plans. Collaborating closely with primary care providers, nurses, and care teams, you’ll support patients with chronic conditions, social stressors, and behavioral health needs. Provide individual and group psychotherapy to adult Medicare patients Conduct comprehensive psychosocial and behavioral health assessments Develop and update evidence-based treatment and care plans Collaborate with primary care providers and interdisciplinary care teams Deliver bilingual (English-Spanish) services and patient education Monitor progress, adjust interventions, and document in the EHR Coordinate referrals to community and social support resources Participate in case conferences, quality initiatives, and team meetings Support crisis intervention and risk assessment as needed Maintain compliance with ethical, legal, and regulatory standards Clinical assessment and diagnosis Individual psychotherapy Group therapy facilitation Treatment planning and documentation Bilingual Spanish-English communication Crisis assessment and intervention Electronic health records (EHR) proficiency Care coordination and case management Motivational interviewing Knowledge of DSM-5 and evidence-based modalities Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LCSW, LCPC, or LMFT license Fluency in Spanish and English Experience in integrated primary care or community mental health preferred Familiarity with Medicare population and chronic disease management preferred #J-18808-Ljbffr
