Registered Nurse (RN) - Home Health Admission - $55+ per visit

Job Title & Location Registered Nurse (RN) Home Health Admission – Macclenny, Florida (coverage of Duval, Baker, Clay and Nassau Counties, including Jacksonville, FL). Responsibilities Follow designated care plans to meet patient needs. Conduct home visits to clients in assigned geographic territories. Administer medication, wound care, treatments, and procedures as directed. Monitor clients’ conditions and report changes to the Clinical or Client Services Manager. Execute and document doctors’ orders accurately. Perform client assessments as required. Coordinate case management and service delivery. Document observations, interventions, and evaluations using the state‑of‑the‑art tablet computer. Qualifications Current Registered Nurse license in Florida. Minimum one year of recent, verifiable clinical experience (medical/surgical). Graduation from an accredited and approved nursing program. Ability to work independently and manage time effectively. Strong interpersonal skills. Solid computer skills; prior electronic medical record (EMR) experience preferred. Willingness to travel to assigned cases.
