Baptist AgeWell Center for Health is seeking a compassionate and experienced Geriatric Physician to join our growing senior-focused primary care team in Clay County, Florida . As part of Northeast Florida's most preferred health system, you'll practice in a collaborative, physician-led environment with extensive support services that allow you to focus on delivering exceptional patient care.

Position Highlights

• Full-time, outpatient senior primary care practice
• Monday through Friday schedule, 8:00 AM - 5:00 PM
• Minimal call responsibilities
• Lead a collaborative care team alongside experienced Advanced Practice Providers
• Epic EMR
• Physician-led, locally governed organization that values physician autonomy

Qualifications

• MD or DO
• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Active or eligible Florida medical license
• Passion for senior care and population health
• Minimum of 5 years of experience in Internal Medicine, Family Medicine, Hospital Medicine, or related practice with senior patients
• Geriatrics experience preferred
