Physician Leader

HarmonyCares

HarmonyCares seeks a Physician Leader, Primary Care to provide home-based primary and palliative care to medically complex, home-limited adults. In this leadership and clinical role, you will carry a panel of patients, conduct in-home and telehealth visits, and oversee interdisciplinary team care plans to reduce avoidable hospitalizations. You will mentor and support clinicians, ensure quality, safety, and regulatory compliance, and champion a compassionate, patient-first culture. Collaborate closely with nursing, social work, and care management to coordinate services, optimize outcomes, and advance our mission in a rapidly evolving value-based care environment.
