Responsibilities
- Conduct OT evaluations for residents and develop individualized treatment plans to address functional goals
- Implement therapy to improve activities of daily living, mobility, balance, and safety with transfers
- Document progress, reevaluate goals, and adjust plans; participate in interdisciplinary rounds
- Educate residents, families, and caregivers on home programs and carryover strategies
- Maintain accurate documentation and ensure compliance with regulatory requirements
- Collaborate with the care team on discharge planning and transitions as appropriate

Requirements
- Active California OT license
- Degree in Occupational Therapy from an accredited program (OTD or MOT acceptable)
- Ability to work in a skilled nursing/long-term care setting
- Strong clinical documentation and communication skills
- Capacity to meet guaranteed 35 hours per week on a day shift
- 13-week travel assignment commitment starting 08/25/2026
