Hospice RN Case ManagerWe are looking for experienced, compassionate, and team-oriented RNs who want to make a profound difference in the lives of patients and families while enjoying a healthy balance between work and personal life.Supportive, positive, team-oriented environment.Meaningful work focused on comfort, dignity, and quality of life.Opportunity to build lasting relationships with patients and families.Work that makes a difference in your community.As a Hospice RN Case Manager, you will:Provide skilled nursing assessment and care in patients' homes and communitiesDevelop and implement individualized plans of careAssess patient and family needs and coordinate appropriate servicesCollaborate with the interdisciplinary group (IDG)Communicate effectively with patients, families, physicians, and team membersMonitor medications, supplies, and equipment for appropriate utilizationDocument care accurately and maintain compliance with hospice regulations and standardsAssist with On call requirementsJob Requires:Current RN license in the State of OhioHospice, home health, or community nursing experience preferredCompassionate, organized, proactive, flexible, and dependableStrong clinical judgment and communication skillsAbility to work independently while being a collaborative team memberA genuine desire to provide exceptional end-of-life careExperience:NURSING: 1 year (Required)License/Certification:CPR Certification (Required)RN License (Required)Ability to Commute:Beachwood, OH and surrounding communities willingness to travel:Work Location:King David CampusSurrounding communities (limited off campus).
