Perform comprehensive patient assessments and develop individualized care plans. Collaborate with physicians and the interdisciplinary hospice team. Provide compassionate education, guidance, and support to patients and families. Maintain accurate and timely documentation in accordance with hospice regulations. Build meaningful relationships with patients and families during an important time in their lives.Active Pennsylvania RN license. Hospice or home health experience preferred. Excellent communication and organizational skills. Compassionate, patient-centered approach.
