Snowline Health is seeking an experienced, compassionate Social Services Director to lead our social services programs and support our interdisciplinary hospice care team. This leadership role offers the opportunity to guide program development, support staff, and ensure exceptional psychosocial care for patients and families navigating serious illness, grief, and end-of-life care.

Employment Type: Full-Time
Schedule: MondayFriday (typically 8:00 AM 5:00 PM; flexibility required), On Call Rotation Required

Position Summary
The Social Services Director provides leadership, oversight, and strategic direction for Snowline Healths social services programs, including hospice and bereavement services. This role ensures delivery of high-quality psychosocial care that aligns with patient goals, promotes dignity, and supports families throughout the care continuum.
Responsibilities include:
- Provide leadership and oversight of social services programs and staff
- Supervise, mentor, and support social workers and related team members
- Ensure delivery of compassionate, high-quality psychosocial care
- Lead program development and continuous improvement initiatives
- Collaborate with interdisciplinary leaders to optimize patient outcomes
- Ensure compliance with regulatory and accreditation standards
- Oversee documentation quality, productivity, and workflow efficiency
- Support staff engagement, training, and professional development
- Partner with community organizations and external agencies
- Assist with complex patient, family, and staff situations
- Promote a culture of compassion, accountability, and excellence
Salary Description

Requirements:- Master of Social Work (MSW) required
- Licensed Clinical Social Worker (LCSW) preferred
- Advanced certification (e.g., ACHP-SW) preferred
- Minimum of 5 years of clinical experience in healthcare, hospice, or related field
- Minimum of 3 years of leadership or supervisory experience
- Experience with program development preferred
- Experience supporting patients and families with grief, loss, and end-of-life care
- Strong leadership, communication, and interpersonal skills
- Ability to work effectively with diverse populations
