OVERVIEW: The Community Coordinator is responsible for the leadership, supervision, and daily oversight of SEMP, Community Habilitation, and Respite Services. The Community Coordinator directly supervises assigned staff and the designated Vocational Lead, manages caseload assignments, ensures required documentation and reporting are completed, and maintains compliance with OPWDD and agency requirements.

The Community Coordinator provides operational leadership across assigned programs to ensure effective service delivery, staff accountability, regulatory compliance, and person-centered outcomes for individuals with developmental disabilities.  

Key Responsibilities

Program Oversight

• Oversee daily operations of SEMP, Community Habilitation, and Respite Services.
• Assign and monitor staff caseloads to ensure appropriate and balanced service delivery.
• Monitor participant enrollment, service delivery, utilization, staffing needs, and program capacity.
• Ensure services are delivered in accordance with OPWDD regulations, Medicaid requirements, Life Plans, Staff Action Plans, and agency policies.
• Collaborate with leadership regarding program performance, staffing needs, service development, and participant outcomes.
• Identify operational concerns and implement appropriate follow-up to maintain effective service delivery.

Staff Supervision & Development

• Directly supervise assigned Job Developers, Job Coaches, Community Habilitation/Respite staff, and the designated Vocational Lead.
• Provide routine supervision, performance management, coaching, and corrective guidance when necessary.
• Monitor staff productivity, schedules, documentation, caseload responsibilities, and service expectations.
• Participate in onboarding, training, and ongoing professional development of assigned staff.
• Ensure the Vocational Lead completes assigned administrative responsibilities while maintaining required direct-service expectations.
• Address staff performance, documentation, attendance, and service-delivery concerns as appropriate.

Participant Services & Documentation

• Participate in Life Plan, Circle of Support, and other team meetings as needed.
• Review, update, and approve Staff Action Plans, Vocational Development Plans, Job Discovery Reports and other required vocational documentation.
• Ensure service notes, collateral contacts, monthly summaries, and other required documentation are completed accurately and within required timeframes.

• Guide staff in determining employment readiness, identifying barriers, developing vocational goals, and implementing appropriate employment strategies.
• Monitor Job Development activity, employer outreach, applications, interviews, placements, and participant progress.

• Oversee required ETP and Job Development reporting, trackers, and program data.
• Provide guidance and escalation support for complex participant or family concerns.
• Monitor EVV compliance , including missed punches and required attestations.

Community Engagement & Employment Support

• Support relationships with employers, volunteer sites, community partners, and training partners.
• Monitor staff efforts related to employment retention, job coaching, community-based skill development, and vocational readiness.

Compliance, Reporting & Quality Assurance

• Ensure services and documentation comply with OPWDD, Medicaid, and agency standards.
• Ensure required monthly and quarterly reports are accurate and submitted timely.
• Monitor program documentation and service delivery for quality assurance and billing readiness.
• Address identified compliance, documentation, or service-delivery concerns with assigned staff.

Additional Responsibilities

• Remain available for urgent staff or participant needs as appropriate.
• Attend required trainings, conferences, supervision, and organizational meetings.
• Serve as a key point of contact for internal and external stakeholders regarding assigned programs.
• Complete additional duties related to program operations as assigned.

Qualifications

• Experience supporting individuals with intellectual and/or developmental disabilities preferred.
• Supervisory or vocational/employment services experience preferred.
• Valid NYS Driver’s License required.
• Must have reliable transportation with proof of insurance and registration.
• Must be computer proficient.
• Smartphone required.
