Hospice Social Worker - Compassionate Family Support
Hospice of the Western Reserve, Inc.
Reserve Care, formerly Hospice of the Western Reserve, is seeking a Social Worker to provide counseling services to terminally ill patients and families in collaboration with the transdisciplinary team across diverse settings. You will support emotional and psychosocial needs, participate in care conferences and bereavement programs, and assist with discharge planning and community referrals, while maintaining records and participating in on-call rotations. #J-18808-Ljbffr Hospice of the Western Reserve, Inc.
