Full time Designated Coordinator Assistant

1. Monitor and assist the development of the Individual’s Intensive Support Services Assessment and Individual Abuse Prevention Plan with the Expanded Team.
2. Monitor and assist to ensure the completion of assessment analysis as needed to obtain individual’s the company supports data.
3. Provide and assist the House Managers with writing and monitoring the implementation of the Individual’s Intensive Support Assessment, Individual Abuse Prevention Plan, Addendums, Behavior Support Strategies and outcomes as needed, or as requested to do so by the Program Administrator.
4. Provides support to House Managers with observing and assessing the progress of the individuals the company supports.
5. As needed, provides individual’s the company supports; assistance, support and supervision to each individual’s support plan.
6. Assists with the summarizing of support plan outcomes and that they are statistically valid.
7. Monitors case management services through the Coordinated Service and Support Plan.
8. Assist with encouraging and providing opportunities for individual’s the company supports choices and presents creative options as needed within the person centered philosophy.

9. Monitors the Intensive Support Assessment, Individual Abuse Prevention Plan, Addendums, Behavior Support Strategies and outcomes

1. Monitors and assists with the development the Program Abuse Prevention Plan.
2. Monitors and assists with ensuring the Residential Emergency Prevention Plan.
3. Assist with encouraging and providing opportunity for individual choices and presents creative options as needed under the person centered philosophy.

13. Maintains all employment standards, required training or licenses. Adheres to the expected work conduct as described in detail in the Employee Handbook.

1. Coordinates and manages the execution of admissions, discharges, closings, transfers and program planning processes as requested.
2. Coordinates a periodic Intensive Support Assessment, Individual Abuse Prevention Plan, Addendums, Behavior Support Strategies and outcomes evaluation process.
3. Develops and monitors the implementation of the Coordinated Services and Support Plan Addendums regularly.

Requirements: 4 YEAR DEGREE/ 10 YEARS EXPERIENCE IN RELATED FIELD

1. Ability to move intermittently throughout the work day.
2. Demonstrated skill in reading, writing, and speaking the English language fluently.
3. Possession of sight/hearing senses, or use of prosthetic devices which will enable these
4. Possess good health and demonstrate emotional stability.
5. Meet licensing requirements of the state or county.
6. Possess knowledge of the procedures, the laws, rules, regulations and guidelines pertaining to the company operations.
7. Possess a valid drivers license.
8. Available to work flexible hours as needed.
9. Ability to maintain positive relations with staff, individuals the company supports and the general public.
10. Ability to supervise the work of professional and non-professional personnel.
11. Ability to plan, organize, develop and implement goals, objectives, policies and procedures.
12. Ability to make independent decisions when circumstances warrant such action.
13. Willingness to take initiative, adapt to circumstances and work independently.
14. Ability to implement any intervention as required.
