Description

• Assess and evaluate patients in inpatient and outpatient environments to create personalized treatment plans
• Deliver physical therapy services that enhance mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Guide patients and families through therapeutic objectives, discharge processes, and home exercise regimens
• Record evaluations, treatments, and patient progress in alignment with facility protocols and regulations
• Engage in interdisciplinary care team discussions and rounds to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications in specialized areas (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Preferred prior experience in similar settings
