Join us and discover a supportive environment where your career can thrive!/ppFamily First Homecare team members represent the organization and its services in a competent, professional and responsive manner while maintaining standards of high-quality patient care and customer service in accordance with all state, local and federal requirements, rules and regulations./ph3Key Responsibilities/h3ulliAccountable for providing care that is based on licensure/certification within the scope of practice./liliPerforms admission, transfer, re-certification, resumption of care and discharge paperwork as assigned./liliMaintains an updated clinical record on each patient at all times, meeting required deadlines for documentation./liliEvaluates the effectiveness of nursing service to the patient and family on an ongoing basis./liliConsults with physicians concerning alterations of patient care plans, documentation of changes in patient care and reports all changes in care to the nurse supervisor./liliSupports FFHCs mission by mentoring and/or coaching new employees during patient specific orientations./liliEducates patients and their caregivers on topics related to patients care needs./liliPerforms supervision for LPNs and Aides in the home as assigned./liliParticipates in FFHC Quality Assurance Performance Improvement program (QAPI) as requested./liliAttends and/or completes all mandatory training/ in-services as assigned./liliSubmits required personnel file documentation in a timely manner./liliAdheres to FFHC compliance and ethics expectations./liliMaintains patient and family confidentiality./liliMaintains professional, supportive, and responsive interpersonal communication skills./liliAssesses physical, functional, psychosocial, and cognitive status of the home care patient utilizing interview observations and physical exam techniques./liliAssesses the home environment for safety and infection control./liliIncorporates multidisciplinary data into the nursing assessment of the home care patient./liliProvides ongoing nursing assessment with the patient, family, and home environment to determine physiologic or psychosocial risk. Applies previous nursing experience and base of knowledge and adapts with the patient and family in a home setting./liliAssesses for the presence of advanced directives and facilitates further action in obtaining information about or implementing advanced directives if indicated./liliParticipates in committees/ meeting./liliPerforms other duties as assigned./li/ulh3Role Qualifications/h3pEducation Experience/pulliRN Nursing Degree. Unencumbered RN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized/lili1 year of demonstrated experience/liliValid driver license with proof of insurance in applicable states/liliClear background screening/liliWorking knowledge of Nurse Practice Acts/liliMust provide valid and current CPR card/certificate that includes in person instructor led hands - on skills check/liliClear Hep B Screening in applicable states/liliClear TB Screening in applicable states/li/ulpState Specific Requirements:/pulliFlorida:/liliIllinois : Valid driver license with proof of insurance: Not required. Staff are prohibited from driving patients./liliHep B Screening: Not a prerequisite for hire. Completed as part of the onboarding process./liliPennsylvania:/liliAdditional Background Screening: Child Abuse, and FBI check./liliMust complete TB screening with 2 step PPD, or 1 step PPD with proof of 2 step PPD administration within the last year./liliTexas :/liliN/A/li/ulh3Key Competencies/h3ulliThe ability to read and communicate in English both verbally and in writing./li/ulh3Working Conditions/Equipment Use/h3ulliWork is performed in the patients home - not substantially exposed to adverse environmental conditions. Some roles at Family First Homecare have a corresponding compensation plan which may include a bonus at rates that vary based on multiple factors set forth in the compensation plan for the role./ppKey Details:/pp** This role requires travel to patients homes to deliver care.
