Work Environment

This full-time position is based onsite at our outpatient office in Lakewood, NJ. You will be part of a collaborative care team that includes physicians, nurse practitioners, physician assistants, medical assistants, and dedicated support staff like medical scribes and registered nurses. Key Responsibilities

• Provide exceptional primary care services, conducting office visits to assess and manage both routine and acute patient issues.
• Administer annual wellness visits and perform thorough health risk assessments, ensuring detailed and precise documentation for optimal patient care.
• Coordinate care with specialists, testing facilities, and other healthcare providers to facilitate comprehensive treatment plans and guide patient pathways.
• Lead population health initiatives, actively ensuring that eligible patients receive necessary preventative screenings and care.
• Assist in phone triage and outreach, responding to patient inquiries and addressing their healthcare needs promptly and accurately.
• Educate patients about their health, treatment options, and care plans, empowering them to take an active role in their wellbeing.
• Participate in promotional outreach activities to enhance community awareness of our services.
• Conduct home visits when needed to assess and support patients in their living environments.
• Report directly to the Medical Director and collaborate with operational leadership to enhance clinic efficiency.

Patient Population / Client Focus

Your primary focus will be on older adults who are enrolled in Medicare. We aim to provide personalized care tailored to the unique needs of this population, helping them achieve their highest quality of life. By fostering long-term relationships and trust, you will play a vital role in guiding them through their healthcare journey.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP Certificate Program graduation.
• Nationally recognized certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from AANP or ANCC.
• Holds an active, non-probationary Nurse Practitioner license in the state of New Jersey.
• Possesses an active DEA license for prescribing controlled substances.
• Demonstrated experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding guidelines, and health risk assessments.
• Strong team spirit with the capacity for cross-functional collaboration.
• Interest in contributing to an innovative population health model.
• Bilingual proficiency is a plus and will be considered an asset.

Preferred Experience

While not mandatory, experience in care coordination and management, understanding of value-based care models, and a track record of working within community healthcare settings will be preferred. Familiarity with electronic health record (EHR) systems is also advantageous.

Compensation and Benefits

The compensation for this position ranges from $120,000 to $160,000 annually, depending on your level of experience and expertise. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
