Job Description

Perform annual home health risk assessments in patient residences throughout Urbandale, Johnston, Clive, and other areas in Polk County, IA. Assess patient health, medical history, and medication use. Educate patients on managing existing conditions and understanding medications. Identify any gaps in current care, recommending actions for improvement. Focus strictly on risk assessmentno requirement to prescribe medications or create treatment plans.

Key Responsibilities

• Perform annual home health risk assessments in patient residences throughout Urbandale, Johnston, Clive, and other areas in Polk County, IA
• Assess patient health, medical history, and medication use
• Educate patients on managing existing conditions and understanding medications
• Identify any gaps in current care, recommending actions for improvement
• Focus strictly on risk assessmentno requirement to prescribe medications or create treatment plans

Qualifications

• Active Nurse Practitioner license in IA
• Master of Science in Nursing (MSN) from an accredited institution
• Board certification through AANP or ANCC
• Current BLS CPR certification
• At least six months of Nurse Practitioner experience (preferred)
