Enhance patient care and impact community health by performing in-depth home health risk assessments for Medicare Advantage plan members throughout Middlesex County. This per diem Nurse Practitioner opportunity offers flexibility and autonomy, with opportunities to serve individuals in Edison, Woodbridge, Old Bridge, and nearby areas. The average home visit is approximately 45 minutes and includes preventive health evaluations or post-discharge assessments without the need for medication prescribing or treatment plan development.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: build your own work calendar
• Minimum weekly or bi-weekly hourly commitment required

Core Responsibilities:

• Complete comprehensive home-based risk assessments for Medicare Advantage members
• Review and document patients medical history, underlying conditions, and medications
• Deliver patient education regarding medical conditions, medication adherence, and healthy living strategies
• Detect care gaps and provide recommendations to close those gaps
• Each visit takes approximately 45 minutes; all visits are conducted in the patient's residence

Qualifications:

• Valid Nurse Practitioner license in New Jersey
• Board Certification (AANP or ANCC)
• MSN from an accredited nursing program
• Active BLS (CPR) certification
• At least 6 months of current experience as a Nurse Practitioner preferred
