Nurse Practitioner for In-home Visits

Job Overview Signify Health, part of CVS Health®, is hiring board‑certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and selected commercial plan members. Key Responsibilities Conduct in‑home visits focused on health and well‑being, including medication and medical history review. Perform a physical evaluation at the member’s home. Complete a pre‑populated, patient‑specific form for each evaluation. Do not prescribe medication, order lab tests, or alter current treatment regimens. Collect data for Pay‑Per‑Test model when applicable. Compensation Base rate: $80 per completed assessment (average 6‑9 visits daily). Potentially increase rate by 135%–160% based on county multipliers. Earn up to $2,500 to $4,000 each week. Additional compensation via pay‑per‑test model for qualifying visits. Schedule & Availability Shifts available Monday–Sunday (no nights or weekends required). Short‑term or long‑term basis. Qualifications Board‑certified nurse practitioner. State license required. Medicaid license or willingness to enroll (with support) preferred. Benefits & Logistics Airfare, hotel, and rental car available for eligible trips. Malpractice coverage provided.
