El enfermero de admisión/evaluación de infusiones es responsable de proporcionar cuidados especializados de terapia de infusión en el domicilio del paciente, realizando evaluaciones clínicas para determinar la idoneidad y la continuidad del tratamiento. Administra terapias intravenosas, educa al paciente y a su familia, y documenta de acuerdo con las normas clínicas, reglamentarias y de acreditación (ACHC, CMS y Departamento de Salud de Puerto Rico). La enfermera de admisión debe fomentar los valores de Alivia, apoyar la visión del departamento de infusión y contribuir activamente al éxito de la unidad.
Responsabilidades:
- Realizar evaluaciones clínicas iniciales en el domicilio para determinar la viabilidad y la seguridad del tratamiento de infusión.
- Admitir a los pacientes en un plazo de 24 horas tras recibir la derivación. De acuerdo con las normas reglamentarias y de acreditación.
- Coordinar la conciliación de la medicación y desarrollar el plan de cuidados inicial en colaboración con el médico prescriptor y el equipo multidisciplinar.
- Supervisar la respuesta del paciente al tratamiento y documentar los hallazgos clínicos en el sistema electrónico.
- Educar al paciente y a su familia sobre el tratamiento, los dispositivos, el control de infecciones, las señales de alerta y los derechos y responsabilidades del paciente.
- Obtener y documentar el consentimiento informado del paciente en el historial médico.
- Identificar y notificar de manera oportuna los eventos adversos, incidentes, quejas, desviaciones o riesgos clínicos al comité de calidad y cumplimiento.
Requisitos:
- Diploma de Enfermero.
- Licencia de enfermero vigente en Puerto Rico.
- Participación en educación continua y competencias clínicas anuales documentadas.
- Licencia de conducir válida en Puerto Rico y disponibilidad de un vehículo motorizado.
- Mínimo de 2 años de experiencia en servicios de salud domiciliaria, infusión o cuidados intensivos. Preferiblemente en un entorno doméstico.
- Conocimientos sobre bombas de infusión y catéteres centrales y periféricos.
