Bilingual Care Navigator for Housing & Wellness
Tucson Police Department
City of Tucson's Housing and Community Development Department is seeking a Care Coordinator to provide in-home and community-based case management to Choice Neighborhood participants and families in shelter and supportive housing programs. You will help connect participants to services, resources, and opportunities to improve health, stability, and housing outcomes. You will collaborate with partners, maintain records, and support relocation and re-occupancy efforts, while adhering to privacy #J-18808-Ljbffr Tucson Police Department
