HIV Care Outreach Navigator

Housing Works

A support organization for HIV prevention is seeking an Outreach Specialist/Patient Navigator in Brooklyn, NY. The role involves leading case finding activities to help clients, addressing healthcare disparities, and providing navigation support to people living with HIV. The ideal candidate will have at least two years of experience in the health field, excellent communication skills, and preferably be bilingual in English and Spanish.
