Job Description

Your Impact:

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patients home.

• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.

• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless safe care transitions.

• Prevent ER Visits: Treat acute needs in the home to keep seniors safe healthy and out of the hospital.

Job Requirements

What You Bring:

• Advanced Degree: Masters Degree (NP) with a preferred focus on Acute Care Adult Geriatric and/or Family.

• West Virginia Credentials: Active unencumbered NP license and Board Certification (ANCC or AANP).

• Reliable Mobility: Valid drivers license and vehicle for local field-based travel.
