What You'll Do

As the Clinical Manager, you'll provide leadership and oversight for all clinical operations while ensuring outstanding patient care and regulatory compliance.

Responsibilities include:

Lead, coach, and mentor Home Health clinicians.
Ensure compliance with Medicare Conditions of Participation (CoPs), state, and federal regulations.
Oversee patient care, clinical documentation, and quality outcomes.
Drive Quality Assurance and Performance Improvement (QAPI) initiatives.
Coordinate interdisciplinary care to ensure exceptional patient outcomes.
Support clinicians with assessments, care planning, and clinical decision-making.
Maintain high standards of regulatory compliance and documentation accuracy.
Promote infection prevention, patient safety, and clinical best practices.
Foster a culture of accountability, teamwork, and compassionate care.

Qualifications Required

Current Registered Nurse (RN) License
Minimum 2 years of Home Health Management experience
Strong knowledge of Medicare Home Health regulations and Conditions of Participation
Current CPR Certification
Valid Driver's License
Valid Auto Insurance
Excellent leadership, communication, and organizational skills

Preferred

HomeCare HomeBase (HCHB) experience
QAPI and Performance Improvement experience
Experience leading interdisciplinary clinical teams
