Clinical Assessment Manager (UAS Required)

Location: Bronx/Manhattan, NY (Orientation in the Bronx)
Setting: Field-based; conducting in-home participant assessments
Schedule: Monday – Friday | 8:30 AM – 5:30 PM (1-hour lunch)
Position Type: Full-Time | Temporary Assignment with Potential for Permanent Hire

About the Opportunity

A leading healthcare organization is seeking an experienced Clinical Assessment Manager to support comprehensive participant assessments in the community. This field-based role is responsible for completing clinical assessments, coordinating participant care, and ensuring compliance with New York State assessment requirements.

The ideal candidate is organized, clinically strong, and experienced in conducting in-home assessments while delivering compassionate, participant-centered care.

Must Haves

• Active New York State Registered Nurse (RN) license
• Hands-on UAS-NY experience (required)
• Experience completing initial, annual, and significant change UAS assessments
• Strong knowledge of comprehensive clinical assessments and care planning
• Experience conducting in-home participant assessments
• Medication reconciliation and medication review experience
• Experience performing post-hospital discharge assessments
• Excellent assessment, documentation, and critical thinking skills
• Valid driver's license and reliable transportation (or ability to travel throughout assigned territory)

Nice to Have

• Previous experience with Managed Long-Term Care (MLTC), PACE, or Home Care
• Care management or case management experience
• Knowledge of Medicare and Medicaid regulations
• Bilingual (Spanish or other languages) is a plus
• Experience working with geriatric or medically complex populations

Responsibilities

• Complete UAS-NY assessments for new enrollments, reassessments, and significant changes in condition
• Conduct comprehensive in-home clinical assessments
• Perform medication reconciliation and medication reviews
• Complete post-hospital discharge evaluations
• Identify participant needs and communicate findings to the interdisciplinary care team
• Ensure accurate, timely, and compliant clinical documentation
• Coordinate with providers, caregivers, and community resources to support participant care
• Maintain productivity and quality standards while providing exceptional participant-centered service
