RN Case Manager, Hospice Full-TimeSuite Living Hospice Care provides compassionate, high-quality hospice services to geriatric patients across the Twin Cities. Our mission is to enhance quality of life by delivering expert, patient-centered care that supports both patients and their families during life's most important moments.Position Overview: We are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10–15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life. This position offers a consistent Monday–Friday schedule, manageable caseload, and a localized territory in the northwest metro, allowing you to build meaningful relationships with patients and families.What You'll Do:Conduct initial and ongoing patient assessmentsDevelop and manage individualized hospice care plansAdminister medications and provide palliative interventionsEducate patients and families on symptom management and care needsProvide emotional support to patients and familiesCare Coordination:Manage and coordinate care for assigned patientsCollaborate with physicians and interdisciplinary team membersParticipate in care conferences and family meetingsAdvocate for patient needs and ensure access to resourcesClinical & Operational Excellence:Maintain accurate and timely clinical documentationSupervise and support hospice aidesEnsure necessary equipment and supplies are in placeParticipate in on-call rotation (5 days/month)Contribute to quality improvement initiativesQualifications:Active RN license in MinnesotaGraduate of an accredited School of NursingCPR certification1+ year of acute care experience requiredHospice or home care experience preferredStrong assessment and clinical decision-making skillsValid driver's license and insured vehicleWhy Join Suite Living Hospice Care?Predictable Monday–Friday schedule with limited on-callManageable caseload (10–15 patients)Localized territory (5–6 communities in the northeast metro)Mileage reimbursement for travelSupportive interdisciplinary team environmentCompetitive pay and comprehensive benefitsOpportunity to make a meaningful impact in patients' livesApply Today If you are a compassionate RN looking to make a difference in hospice care, we'd love to connect with you.
