Hospice RN Case ManagerThe Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of patients in accordance with agency standards and policies. The Ecumen Hospice Twin Cities team focuses on providing life honoring care for patients. The Ecumen Hospice mission is to provide care that exceeds the expectations of those that find themselves in the later stages of life by providing a level of comfort and quality of life that makes each person feel honored and supported as their life continues to have significance.Schedule: Full-Time 80hrs bi-weekly M-F 8am-4:30pm occasional on-call requirements - 1 weekend per month and 3 holidays per year.Pay: The targeted pay range for this job is $40.00-$45.00 per hour. Provides a complete physical assessment and history of current and previous illness.Collaborates with physicians, other health care professionals, patients, and families in developing a comprehensive coordinated plan of care.Develops plan of care which includes objective goals for patient's end of life care. Evaluates plan of care on continuous basis and updates physicians of changes in patient status and need for revision of plan of care.Ensures that physician orders are communicated to all members of the care team with accuracy and thoroughness and understanding.Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.Promotes personal safety and a safe environment for clients and co-workers.Driving Requirements: Local Travel RequiredMinimum Required Qualifications:Associate's Degree in NursingMust be licensed as a Registered Nurse within the state in which working and must maintain licensure.Ability to communicate effectively both verbal and writtenProficient with Microsoft Office SuiteAbility to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulationsPreferred Qualifications:Bachelor's degree in NursingPrevious experience working with seniors1 year of nursing experience in post-acute care settingTravel Required: Yes. Travel required through the Twin Cities area to visit patients.
