Job Description

Job Title: Hospice CNA | The Heart of Our Care Team

Compensation: $19-$25/hour DOE + mileage reimbursement paid by wage + $500 Sign-On Bonus

Benefits: Full benefits package available for full-time staff

Schedule: Per Diem or Full-Time | Weekend rotation

Location: 21-County Twin Cities Service Area

What You'll Do

Assist patients with bathing, dressing, grooming, toileting, and mobility.
Monitor patient comfort and report changes in condition to the nursing team.
Provide companionship and emotional support to patients and families.
Help create a safe, clean, and comfortable environment.
Follow individualized care plans while working closely with nurses and the interdisciplinary team.
Deliver care that promotes dignity, comfort, and quality of life.

Who We're Looking For

Current Minnesota CNA certification.
Hospice experience preferred, with home care, assisted living, long-term care, hospital, or oncology experience also considered.
Compassionate, observant, and dependable.
Excellent communication and collaboration skills.
Valid driver's license and reliable transportation.
