**Join Our Compassionate Hospice Team as a Case Manager RN!** - Are you a skilled RN looking to make a profound impact in your community? - Do you thrive in a supportive team environment and want to provide exceptional home care? - If so, we have the perfect opportunity for you! **Job Description:** - Provide skilled nursing and coordinate care for patients and families in their homes. - Conduct assessments, evaluations, and plan interventions based on the patients' physical and psychosocial needs. - Oversee and support Home Health Aides and CNAs, ensuring compliance with care plans. - Ensure continuity of care through effective communication with interdisciplinary teams, community health, and support agencies. - Deliver quality care and symptom management, promoting optimal health and quality of life through education and referrals. - Maintain accurate and up-to-date patient records in line with agency standards and OASIS requirements. - Participate in scheduling coordination and on-call rotation to meet patient care needs via phone consultations and home visits. - Engage in quality improvement activities and attend in-service training sessions as assigned. - Perform additional duties as required by your supervisor. **Minimum Qualifications:** - Current RN license in the State of Montana (BSN preferred). - Valid driver’s license and reliable transportation. - 2 years of recent experience in acute care, home care, or hospice preferred, including skills such as venipuncture, catheterization, and wound care. - HCHB experience is strongly preferred. **Shift Information:** - Typical schedule: Monday to Friday, 8 AM to 5 PM, with on-call and weekend rotation as necessary. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
