Job Opportunity

We are seeking a Registered Nurse for a Home Health assignment. This role requires providing care for patients in their homes and attending periodic office meetings.

Contract Details

Duration: 13 weeks
Schedule: Monday to Friday, 8:00 AM to 5:00 PM
Weekend Rotation: 1 Saturday per month, 8:00 AM to 5:00 PM
On-Call Duties: None
Territory: Erie County, NY
Travel Expectation: Approximately 250 miles per week

Responsibilities

Conduct patient home visits with an average daily caseload of 5 to 8 patients.
Complete patient assessments, care management, and documentation in accordance with home health guidelines.
Meet weekly productivity expectations of 30 points, equating to full-time hours.
Document all patient interactions and care within 24 hours.
Travel to patients' homes for care as required, with the understanding that some drives may be longer than 20 minutes.
Provide various patient care services including wound care, follow-ups, case management, and more.

Qualifications

Minimum of one year of recent adult home health or hospice experience.
Active RN license in hand (pending licenses are not accepted).
CPR and BLS certifications are required.
Proficiency in Homecare Homebase (HCHB) EMR is required, with recent experience preferred.
Ability to travel up to 75 miles from the assigned care center.
Reliable transportation along with a valid driver's license and appropriate auto insurance.

Working Conditions

No guaranteed hours; productivity and patient census may vary.
Potential to float to nearby clinics for additional hours if patient load is low.
All travel and mileage will be compensated at the standard rate.

Additional Information

The ideal candidate must understand the responsibility of driving to patients' homes daily and keeping up with documentation deadlines. Acceptance of all scheduled patients, regardless of location, is expected. This opportunity offers valuable experience in home health care.
