Participant Care Coordinator – St. Paul’s PACE San Diego’s St. Paul’s PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program that supports individuals 55 years or older who are living with chronic illness or disabilities. The healthcare teams provide a hands‑on approach coordinating medical, social, and home care services so individuals can continue living as independently as possible in their home and community. Hours: Monday – Friday 8:00 AM to 4:30 PM Job Summary: The Participant Care Coordinator (PCC) schedules participant appointments across various services—including medical, dental, therapy, specialty care, and transportation—ensuring that ordered appointments are completed within participants’ individualized care plans and regulatory requirements. PCCs support the interdisciplinary care team (IDT) and maintain continuous communication with participants, interdisciplinary staff, and contracted providers. Duties and Responsibilities Schedules ordered appointments within the regulatory timeframes set by DHCS and CMS Accurately documents necessary information in participants’ medical records and performs electronic entries across multiple platforms as established by department protocols Coordinates follow‑up appointments and works collaboratively with the medical records department to ensure providers and clinic staff have pertinent information to care for participants in a timely manner Prepares packets and documentation for specialty visits Attends the daily IDT meetings Communicates appointment details and changes to participants, caregivers, and IDT members Answers department inbound phone calls via call queue Maintains scheduling‑related key performance indicators (KPIs), including appointment scheduling timeframes, rescheduling turnaround times, telephone call queue times, and overall appointment coordination efficiency Maintains accurate records and ensures compliance with privacy regulations Performs other duties as assigned Qualifications High School Diploma or equivalent required Minimum of one year of administrative support experience Benefits Flexible Vacation Time – 17 days Paid Sick Time Education reimbursement – up to $3,000 per year Qualified Public Student Loan Forgiveness (PSLF) employer 403(b) with match Child care available Opportunities for upward mobility Security and Background Checks YOU MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.
