Occupational Therapist (OT) | Temporary Full-Time | $55–$70/hr | Tracy, CA | May 4 – June 12 Tracy Nursing and Rehabilitation Center is seeking a licensed Occupational Therapist (OT) for a temporary full-time assignment at our Skilled Nursing Facility (SNF) in Tracy, California. This is a short-term coverage opportunity running from May 4 through June 12, ideal for a travel OT, per diem therapist, or licensed Occupational Therapist looking to pick up immediate full-time hours in a long‑term care and post‑acute rehabilitation setting. Position Summary Provides therapy and rehabilitation services by evaluating patient abilities and creating individualized treatment plans with appropriate therapeutic equipment and exercises in accordance with education, training, current applicable federal, state, and local regulations, company policies, and as directed by the Therapy Director or facility Administrator to assure the highest degree of quality care and results are delivered. Position requires Current or Pending California Occupational Therapist (OT) State Licensure Responsibilities Review patients’ medical history, ask the patients questions, and observe them doing tasks Evaluate a patient’s condition and needs Develop a treatment plan for patients, identifying specific goals and the types of activities that will be used to help the patient work toward those goals Help people with various disabilities perform different tasks Demonstrate exercises that can help relieve pain in people with chronic conditions Evaluate a patient’s home or workplace and, on the basis of the patient’s health needs, identify potential improvements Educate a patient’s family and employer about how to accommodate and care for the patient Recommend special equipment and instruct patients on how to use that equipment Assess and record patients’ activities and progress for patient evaluations, for billing, and for reporting to physicians and other healthcare providers.
