Case ManagerThe Case Manager provides exceptional patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program. This role acts as a liaison between the patient, family, nursing staff, physicians, multidisciplinary teams, and patients' healthcare benefactors to optimize outcomes.Key ResponsibilitiesServe as a consultant to the healthcare team on specific patient needs.Coordinate the continuum of care for a defined population of patients from pre-admission through post-discharge.Ensure that patients and families navigate efficiently through the course of hospitalization.Patient Care FocusThis role involves working with a diverse patient population diagnosed with acute psychiatric, behavioral, or mental health disorders, providing evaluation and treatment tailored to the needs of patients of all ages.QualificationsStrong communication and interpersonal skills to effectively collaborate with various stakeholders.Demonstrated ability to manage complex care processes and optimize clinical outcomes.Experience in case management or related healthcare field is preferred.LocationPosition is available in City, State.
