Elliot VNA – Home Health & Hospice Liaison is seeking a proactive sales/marketing professional to build, establish, and maintain strong referral relationships with SNFs, physicians, discharge managers, and community partners, ensuring patients access VNA services. You will coordinate transitions to the agency, provide in-services about home health and hospice care, promote programs, and help grow referral volume while working with the Director on service expansion. #J-18808-Ljbffr Elliot Health System
