About the Role:

As an LPN, you will work closely with our interdisciplinary team to provide high-quality, patient-centered care to our residents. Your responsibilities will include:

• Providing direct patient care, including administering medications, performing vital signs, and documenting patient information

• Assisting with daily living activities, such as bathing, dressing, and grooming

• Monitoring and reporting changes in patient condition to the nursing team

• Collaborating with colleagues to develop and implement individualized care plans

• Maintaining a clean and safe environment, adhering to infection control and safety protocols

Requirements:

• Current LPN licensure in the state of Pennsylvania

• Minimum of 1 year of experience in long-term care or rehabilitation setting
