Registered Nurse Clinical Manager – Home Health

Job Type: Full-Time
Location: Boynton Beach, FL

Job Overview:

We are seeking an experienced and dedicated Registered Nurse Clinical Manager to oversee clinical operations and ensure high-quality patient care in a home health setting. The ideal candidate will have hands-on Medicare Home Health experience, strong clinical leadership skills, and a background in supervising nursing staff and coordinating patient care.

Key Responsibilities:

• Conduct patient assessments and reassessments to determine care needs and eligibility for services.
• Develop, review, and update individualized patient care plans.
• Evaluate admission criteria and oversee patient admissions, recertifications, and resumptions of care.
• Coordinate patient care and assign cases to appropriate clinical staff.
• Monitor patient progress and ensure therapeutic goals are met.
• Supervise, train, and support clinical staff to maintain quality standards.
• Participate in staff interviews, onboarding, training, and performance evaluations.
• Review start-of-care, resumption-of-care, and recertification documentation for accuracy and compliance.
• Promote effective communication and coordination among patients, families, caregivers, and healthcare professionals.
  

Required Qualifications:

• Graduate of an accredited School of Nursing.
• Current and valid Registered Nurse (RN) license in the applicable state.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Required: Medicare Home Health experience.
• Minimum of 2 years of RN experience, including at least 1 year of management experience in home care, hospice, or an equivalent setting.
• Strong clinical assessment, care coordination, leadership, and communication skills.
