Job Overview

We are seeking an experienced and dedicated Registered Nurse (RN) Clinical Manager to oversee and coordinate direct patient care services in a home health setting in Boynton Beach, Florida. This role is responsible for ensuring high-quality patient care, regulatory compliance, accurate clinical documentation, and effective coordination among clinical staff, physicians, patients, and caregivers.

The ideal candidate will have strong clinical leadership skills, hands-on Medicare Home Health experience, and proficiency with OASIS assessments and documentation requirements.

Key Responsibilities

• Coordinate and oversee direct patient care services provided by clinical personnel.
• Conduct and delegate patient assessments and reassessments, update plans of care, and interpret patient needs in accordance with company policies, physician orders, and applicable healthcare regulations.
• Ensure patients meet admission criteria and determine eligibility for home health services.
• Assign appropriate clinicians to patient cases based on patient needs and care requirements.
• Assist clinicians in establishing short-term and long-term therapeutic goals, setting priorities, and developing individualized Plans of Care (POCs).
• Coordinate communication among clinical team members, attending physicians, patients, and caregivers to support effective care delivery and positive patient outcomes.
• Supervise assigned clinical employees and provide direction, oversight, and performance management.
• Participate in interviewing, hiring, training, and developing clinical staff.
• Evaluate employee performance, provide coaching, recommend in-service education, and ensure adherence to company policies and clinical standards.
• Review clinical documentation, including Start of Care (SOC), Resumption of Care (ROC), and recertification assessments, to ensure appropriate care and compliance with documentation requirements.
• Ensure compliance with Medicare Home Health regulations, including OASIS (Outcome and Assessment Information Set) assessment, documentation, and submission requirements.
• Promote consistent clinical quality, regulatory compliance, and accurate patient records.

Required Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the applicable state.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Required: Hands-on Medicare Home Health experience , including OASIS assessments, documentation, and regulatory compliance.
• Minimum of two years of Registered Nurse experience.
• At least one year of management experience in home care, hospice, or an equivalent healthcare setting.
• Strong clinical assessment, leadership, communication, and organizational skills.
• Ability to coordinate multidisciplinary care and oversee clinical personnel effectively.
