REQUIREMENTS
• RN- AZ- license in good standing
• 2 years of current acute care/sub acute/Long-term experience
• BLS/ACLS/NRP (front/back-singed)
• 2- Current references (RN's)
• Current - Physical (within-12 months)
• Current - PPD (within- 12 months)
• Titers- MMR/Varicella/Hep B
• Tdap and Flu vaccine - current
• Respiratory Fit testing (within 12 months)
• Must be able to pass a back ground screening
• Must be able to pass urine drug testing
