Sentara Health Plans is hiring an Integrated Nurse Case Manager/Registered Nurse (RN) for chronic/complex conditions in Fredericksburg, VA, with travel to surrounding areas. This is a full-time, work-from-home role requiring 2–3 face-to-face home visits per week. Ideal candidates hold an RN license (VA or compact) and have 3+ years of nursing experience, including case management.
