Remote RN Case Manager

Working remotely in a full-time capacity, the Remote RN Case Manager will manage patient care by building relationships, creating personalized care plans, and coordinating services to support patients' medical, behavioral, and social needs. Key responsibilities Building meaningful relationships with patients and families to understand their goals and barriers Creating personalized care plans that address social determinants and emotional well-being Coordinating care across providers and settings to ensure safety, quality, and continuity Required qualifications Minimum of 5 years as a Registered Nurse Active RN license: compact, multi-state licensure Strong background in care management and complex chronic condition management Skilled in motivational interviewing, patient education, and patient advocacy Comfort with technology and multiple EMR systems
