What You'll Do
- Provide outpatient and inpatient physical therapy services
- Assist patients of all ages, including children in local schools and home health clients
- Document patient progress using Epic EMR system
- Collaborate with multidisciplinary teams to develop personalized care plans
- Support facility's mission to serve the community with high engagement and care standards
What You Need
- Valid Minnesota Physical Therapist license
- Basic Life Support (BLS) certification (within 1 year of hire)
- At least 1 year of relevant clinical experience
- Strong knowledge of electronic medical records, preferably Epic
- Key skills: patient-focused care, teamwork, adaptability
