We are seeking a compassionate and dedicated registered nurse specializing in hospice and palliative care to join a supportive healthcare team in the Midwest region. This role focuses on delivering exceptional end-of-life care, ensuring patients' comfort, dignity, and quality of life are at the forefront. The nurse will provide care in home settings or hospice facilities, tailoring individualized care plans and collaborating with a multidisciplinary team to meet each patient's unique needs.

Key Responsibilities

• Perform comprehensive assessments addressing physical, emotional, and spiritual needs of patients.
• Conduct initial physical evaluations and ongoing patient assessments.
• Develop and implement patient-centered care plans in partnership with healthcare professionals.
• Administer medications and manage symptoms following established protocols.
• Educate and support patients and families about end-of-life care options and processes.
• Coordinate services and resources to assist patients and families throughout the hospice journey.
• Maintain accurate and timely documentation of patient care activities and updates.
• Communicate changes in patient condition to the interdisciplinary care team.
• Perform venipuncture or collect lab specimens as ordered by physicians.
• Notify pharmacy and patients regarding medications and supplies related to hospice care.
• Keep primary and consulting providers informed about patient status.

Education & Experience

• Graduate of an accredited nursing program with current registered nurse licensure valid in the state of Iowa or eligible to work there.
• Previous experience in hospice, palliative, or critical care settings is preferred.
• Completion of hospice philosophy and care training is required.

Requirements

• Current basic life support (BLS) certification per American Heart Association standards.
• Completion or commitment to complete mandatory reporter training within six months of hire.
• Adherence to all federal, state, and professional licensure and regulatory training requirements.
• Strong oral and written communication skills.
• Basic computer proficiency and ability to perform simple mathematical calculations.
