Registered Nurse - Dialysis (Home Hemo & Peritoneal)

Role Summary The Home Therapy Nurse trains and supports patients in self-administering dialysis at home, ensuring adherence to federal, state, and organizational regulations. They conduct home assessments, provide patient education on peritoneal or hemodialysis, and document all nursing activities in electronic records. The nurse collaborates with physicians, monitors patient progress, manages supplies, and participates in quality improvement initiatives to optimize patient outcomes. They also handle emergency preparedness, infection control, and medication management, including routine lab tests and medication reviews. Qualifications & Skills Current RN license in applicable state with good standing Minimum 12 months of clinical nursing experience; 3 months in dialysis modality (6 months in some states) CPR certification and ability to distinguish primary colors Completion of Home Therapy Nurse Orientation and ongoing competency maintenance Strong communication and computer skills; proficiency in clinical applications within 90 days Ability to perform home evaluations, emergency procedures, and supply management Key Responsibilities Train patients and caregivers in dialysis self-care Perform home visits and assess home environment Monitor patient health, communicate with healthcare team, document care Manage supplies, labs, medications, and emergency protocols Participate in quality improvement activities and on-call duties Ensure compliance with regulations and maintain professional standards #J-18808-Ljbffr CONSUMER DIRECTED SERVICES INC.
